Separate consent for a reply to your question, business inquiry or requested callback.
01. Controller and action
I freely consent to processing by Evotech-Mirai Genomics LLC, taxpayer number 7728364310, registration number 1177746213512, address: 422624, Russia, Republic of Tatarstan, Laishevsky District, Bolshie Kabany, Vystavochnaya Street, building 1A, room 1000.
I express this consent by selecting the initially unchecked consent box and submitting the inquiry. The server rejects submission without it. Optional analytics and external-media permissions are independent and can remain off.
02. Purpose and data
The purpose is to receive and consider my inquiry, answer a question about the company or products, discuss a business proposal or make the callback I requested. Data includes the name, telephone number and/or email address, message and relevant organisation or role details I voluntarily provide.
I will not send medical results, diagnoses, genetic data, identity documents or excessive third-party details through this form. This consent does not cover special-category data, medical procedures, advertising or public disclosure.
03. Processing and recipients
I allow necessary collection, recording, organisation, storage, correction, retrieval, use for contact, necessary disclosure to authorised recipients, restriction, deletion and destruction, using automated systems and authorised employees.
Recipients are staff who handle inquiries and appropriately engaged hosting and corporate-email providers within their assigned responsibilities and confidentiality obligations. Inquiry contents are not sent as parameters to Yandex Metrica, Google Analytics, maps or video players. This is not consent to advertising.
04. Duration, withdrawal and evidence
Consent lasts until the inquiry and necessary correspondence are completed, or until withdrawal. Where no contract or other lawful retention basis exists, the data must be deleted within 30 days after the purpose is achieved or withdrawal is received, as required by law. Withdraw by writing to mail@evotech-mg.com with subject “Inquiry consent withdrawal”, or to the controller’s postal address. Provide the submission date and contact used to help locate the inquiry.
Withdrawal does not invalidate previous lawful processing or independently lawful processing, such as contractual or statutory duties. A minimal receipt containing a random number, form purpose, text revision and time is kept for up to three years for evidence and rights protection. It contains no message or contact details. I can request information, correction, restriction or deletion and complain to Roskomnadzor or a court as explained in the personal data policy.
